1. Completed, printed and signed visa application form in the e-konsulat system;

6. A copy of an identity document confirming legal residence in the consular district of the institution where you are applying for a visa.
7. Confirmation of the availability of financial resources in the form determined by the object.
8. Proof of accommodation (for example, an official invitation or hotel reservation).
9. Documents confirming the purpose of stay on the territory of the Republic of Poland.

Additionally, when applying for a visa for minors, you must provide:
Written notarized consent of the parent / parents to apply for a visa. If there is only one guardian, this must be documented by presenting a birth certificate, court order granting sole parental responsibility, or death certificate of the other parent;
Parents’ passports — original and copy;
Birth certificate — original and copy.
If the child is entered in the passport of a parent or legal guardian, submit a separate visa application. The visa will be glued into the passport of the parent or legal guardian.
Remember:
You must apply for a visa at a diplomatic mission whose consular district is your permanent residence.
As a rule, the above documents are sufficient to make a decision, but the consul may request additional documentation.
The consul has the right to invite the applicant for an interview at the office, but is not obliged to do so.
D type visas allow you to stay in other Schengen countries for 90 days in any 180 day period. This means that you can legally stay in the Schengen area only if your stay in the Schengen countries does not exceed 90 days in the last 180 days.
A type D visa can be issued for a maximum duration of one year.
Remember that a visa does not guarantee entry into the territory of Poland — the final decision is always made by the border service.

The procedure for obtaining a residence permit of the Slovak Republic:
1. After providing the documents and paying the agent, the employer submits an application to the Migration Police of Slovakia to verify the identity of the candidate for problems with the law in the EU.
2. The Slovak police submits a request to the EU police and the consulate of the Slovak Republic in the applicant’s country of citizenship to confirm the possibility of obtaining a residence permit, no criminal record and restrictions on the applicant’s departure from the country of citizenship.
* If the police of the Slovak Republic and the police of your country of citizenship have information about the prohibition of issuing a residence permit (based on violations of EU legislation or legislation in the country of citizenship), the applicant’s application is canceled and the advance payment is not refundable!
3. The employer prepares a work contract for the applicant. The contract is provided from a recruiting agency in Slovakia with the possibility of working at the Eissmann car factory or the ALZA logistics company in the Slovak Republic.
4. After checking by the Slovak police, the applicant is given:
— A copy of the invitation, certified by the migration police
— A copy of the document confirming the provision of accommodation
This is the basis for the final settlement with the agent.
After paying the second part of the cost, the applicant is provided with color scanned copies of the invitation, registration of residence, a bank statement, which confirms the applicant’s financial support by the inviting party.
The applicant contacts the Slovak consulate in the country of filing and clarifies whether he can apply for a visa, the name is only a scanned copy of the documents. After that, the employer contacts the consulate to resolve issues with the submission of documents.
If the consulate requires the provision of original documents, then the applicant must pay the shipping cost and provide the address of receipt of the original documents.
6. While the documents are delivered to the applicant, he draws up additional documents that are required to submit an application to the consulate (photos, insurance policies, etc.). The applicant can find detailed information on the required documents on the website of the Slovak consulate in his country of citizenship in the «Residence permit» section.
7. After confirmation of the application by the consulate, the applicant submits documents and expects to receive a visa. If the applicant has difficulty getting the queue or there is a delay, then you must contact the agent to resolve this issue. The service to resolve this issue is provided by the agent free of charge.

Let me explain why: The applicant does not need to prove that he will return to his country of citizenship after the expiration of the visa, that he will not become an illegal migrant. On the basis of these documents, the consul understands that the applicant is leaving forever and in the EU he will definitely receive legal documents for life and work. A refusal can only be issued if the applicant has incorrectly prepared his documents.
8. The applicant receives a category D national visa for a period of 30 to 90 calendar days. This period is enough to have time to get a residence permit in Slovakia.
9. The applicant solves the issues with the flight to Slovakia and he is provided with information about who and where will meet him in Slovakia.
10. After arrival and meeting, the applicant is provided with temporary accommodation (in some cases, it can cost about 5 euros per day
11. The applicant, together with the coordinator, within the first 3 working days after arriving in the territory of the Slovak Republic, are sent to the migration service, where they sign an application for issuing a residence permit card, the applicant is photographed on the card and he will provide his fingerprints.
